5 Silent Warning Signs Your Heart Could Be in Trouble

Heart disease is often called a “silent killer” because its warning signs can be subtle or easily mistaken for everyday problems. While these symptoms don’t always mean you have heart disease, they shouldn’t be ignored—especially if they are new, persistent, or worsening.

Here are five signs that may indicate your heart needs medical attention.

1. Swollen Feet, Ankles, or Legs

One of the most overlooked warning signs is swelling in the lower legs or feet.

When your heart isn’t pumping blood efficiently, fluid can build up in your body’s tissues, causing swelling, also known as edema.

Watch for:

  • Puffy ankles or feet
  • Shoes feeling tighter than usual
  • Swelling that worsens throughout the day
  • Skin that stays indented after pressing on it

Occasional swelling can have many causes, but persistent or unexplained swelling should be evaluated by a healthcare professional.

2. Shortness of Breath

Feeling breathless after climbing stairs is normal. However, becoming short of breath during simple daily activities—or while resting—may be a warning sign.

This can happen when the heart struggles to pump blood effectively, allowing fluid to back up into the lungs.

You may notice:

  • Difficulty catching your breath
  • Waking up at night feeling breathless
  • Needing extra pillows to sleep comfortably
  • Fatigue with minimal activity

3. Chest Pressure or Discomfort

Heart problems don’t always cause severe chest pain.

Many people describe the sensation as:

  • Squeezing
  • Fullness
  • Burning

The discomfort may come and go and can spread to the:

  • Arm
  • Shoulder
  • Neck
  • Jaw
  • Back
